-Agree that the sole purpose I am applying for a User ID and password (“credentials”) on Research Expense Organizer, LLC’s website is to complete the R&D Checklist;

-Agree that my credentials will be my electronic signature, and when my credentials are used to create, generate or send the R&D Checklist, they will identify that record or document as mine;

-Understand that a record or signature may not be denied legal effect or enforceability solely because it is in electronic form;

​

-Agree that because an electronic record will be created with my credentials will be attributed to me, I must keep my credentials secure, and I will not disclose my credentials to another person or allow another person to gain access to my credentials;

 

-Agree that the laws of the Commonwealth of Pennsylvania shall apply to the use of this website and all transactions with Research Expense Organizer, LLC, without regard to its choice of law provisions;

 

-Agree irrevocably and consent to the sole jurisdiction and venue of the Court of Common Pleas of Blair County, Pennsylvania for any legal action arising from the use of this website and/or any transaction with Research Expense Organizer, LLC, including enforcing any right I may have under any federal statute or regulation, and I waive all objections to personal jurisdiction and venue; and

 

-WAIVE MY RIGHT TO A TRIAL BY JURY, AND AGREE AND CONSENT TO A TRIAL BY COURT.
